Scheduling with a provider
The scheduling tool is for existing patients only. Please make sure to schedule only with your provider.
If you are a new patient, please contact us here to request an intake form.
Rescheduling or canceling an appointment
If rescheduling, please use the reminder e-mail or text from Yellow Schedule to cancel your existing appointment. To do so, simply click "No" on the reminder e-mail or reply "No" to the reminder text. (Even if you already said "Yes")
If you need to cancel, please give us a 24 hour notice to avoid the missed appointment fee of $75.
